In SafetyReports, you can track corrective measures by adding Action Items to an existing incident record. This is completed within the 5 Why's analysis section to ensure all root causes are addressed.
Note: Prior to adding an action item to an Incident, the Incident needs to be submitted.
Submitting an Incident (Web App)
Under the Recent Incident column, select the Incident you want to submit.
Under the Incident column, select the arrow at the bottom of the window and navigate to the last page of the Incident report.
Under the Incident column, review the report information and click the Submit Incident button at the bottom of the window.
Submitting an Incident (Mobile App)
From the home screen, select the Incident you want to submit.
Select the arrow at the bottom of the page and navigate to the last page of the Incident report.
Review the report information and click the Submit Incident button at the bottom of the page.
Adding Action Items
Select Incidents from the top menu.
Select Incidents from the left-hand menu.
Click the Edit icon next to the specific incident you wish to update.
Click the 5 Why's tab at the top of the page.
Enter the details of the issue in the Problem Statement field.
Scroll to the Action Items section at the bottom of the page to add and save your items.
