Corrective actions are essential in resolving safety incidents effectively. This guide provides a step-by-step reference for responsible parties tasked with recording remediation steps in the Safety Reports Incident System. It outlines how to properly document actions taken to resolve issues identified in the incident investigation, including the use of the 5Why process.
Receiving an Email from Safety Reports
As a responsible party, you will receive an email notification from the Safety Reports system when corrective actions are assigned to you by an account administrator. The email, sent from reports@safety-reports.com, will include incident details and the corrective action items assigned to you. These items are part of the 5Why follow-up process, where you’ll be required to provide remediation updates.
To begin recording your actions, click on the ‘Update Action Item’ link in the email. This will direct you to the corrective action details page, where you can enter information regarding the steps you’ve taken.

Clicking the link will open a new page in your web browser. On this page, you’ll find key details about the incident, such as any photos and the specific ‘Question’ associated with the corrective action within the 5Why process.
Scroll down to the “Action Entry” section, located below the photo section. Here, you will record the actions you’ve taken to address the incident.
Action Entry – Recording Your Actions
There are two statuses you can assign to the corrective action follow-up: ‘Pending’ or ‘Closed’. The status depends on the progress made in resolving the issue. Here’s how to enter the correct status:
‘Taken/Complete’:
If the corrective action has been fully addressed, select the ‘Taken/Complete’ radio button. In the text field, provide any relevant details about the remediation and the steps taken to resolve the issue. This action will mark the follow-up as Closed.‘Requested/Planned’:
If you’ve addressed the corrective action but further steps are needed, select the ‘Requested/Planned’ radio button. This will set the follow-up status to Pending, indicating that additional actions are still required.
For both options, ensure that you provide relevant details about the actions taken in the text field. It’s important to record your name or initials to show accountability. Additionally, use the date/time box to enter the actual date and time the action was taken (you can backdate it if needed).
Submitting Your Updates
Once you’ve entered the corrective action details and chosen the appropriate status, click ‘Submit Updates’ to save your changes. This will update the system with your remediation progress and ensure that the incident resolution process moves forward.
By following these steps, you can efficiently manage and track the corrective actions assigned to you within the Safety Reports Incident System, helping to ensure incidents are resolved and safety standards are maintained.
