These instructions are intended for those using the mobile version of our Safety Incident app. If you haven't already done so, please download the app (Android | Apple) to get started. The Incident app is compatible with both smartphones and tablets. Our app is also accessible from your computer via our website.
Getting Started
Use your credentials to sign into the app from the Settings tab. After signing in, past incidents will be displayed on the incident home screen. Color coding allows you to differentiate between submitted and in-progress incidents.
Creating a New Incident
Tap the green “Start New Incident” button.
Enter an Incident Name and select Incident Type (Vehicle in this case) from the drop-down menu.
Additional fields may be helpful, such as:
Employee or 3rd Party Involvement
Location Selection (Prefilled by your administrator or entered manually)
Activity Type (e.g., normal activity, overtime, etc.)
Note: Some incident types allow Geo-Tagging after selections are made.
Describing the Incident
Use the provided fields to detail:
What led up to the incident
A narrative description of the incident itself (this becomes the Incident Description on the report)
Note: Certain accounts may have custom fields that must be completed before submission. Required fields will have an asterisk (*).
Entering Vehicle Incident Details
Provide details about your vehicle, including:
Vehicle type
Actions at the time of the accident
Descriptions and/or photos of damages
If other property was damaged (e.g., vehicles, buildings), document it in the Other Property Damage section.
Add relevant photos of property damage
Ensure these photos are specific to property damage as there is another section for general incident photos
Documenting Site Conditions
Use drop-down menus and a narrative field to describe the accident site conditions.
Complete the Vehicle Accident Diagram:
Add images of vehicles, construction equipment, or other objects
Rotate and adjust images as needed
Use free-hand illustration for additional details
Recording Unsafe Driving Behaviors
Use checkboxes to indicate any unsafe driving behaviors
Provide additional details in the text field if necessary
Adding Photos
Capture photos using the camera feature or upload from your device’s gallery
Note: There is a separate section for property damage photos related to Vehicle incidents
Witness Information
If there were witnesses:
Tap the green “Add Witness” button.
Enter the witness’s details.
Record an audio statement (optional).
Capture the witness’s signature to include in the report.
By following these steps, you ensure accurate and thorough documentation of vehicle-related incidents, aiding in effective reporting and resolution.
