These instructions are intended for those using the mobile version of our Safety Incident app. If you haven't already done so, please download the app (Android | Apple) to get started. The Incident app is compatible with both smartphones and tablets and is also accessible from your computer via our website.
Getting Started
Use your credentials to sign into the app from the Settings tab. After signing in, past incidents will be displayed on the home screen. Color-coding helps distinguish between submitted and in-progress incidents.
Creating a New Incident
Tap the green “Start New Incident” button.
Enter an Incident Name.
Select Incident Type from the dropdown menu. Available types include:
Injury
Illness
Near Miss
Vehicle
General Liability
Property Damage
Indicate if the incident involves an Employee or a 3rd Party.
Select a Location from predefined options or manually enter the location.
Use the Activity dropdown to specify the activity at the time of the incident (e.g., normal activity, overtime).
If applicable, enable Geo-Tagging to capture the incident location.
Use the black arrow keys at the bottom of the screen to navigate between sections.
Incident Details
On the details screen, describe:
What led up to the incident
A detailed narrative of the incident
This narrative will appear as the Incident Description on the report. Custom fields may also be present based on account settings, marked with an asterisk (*) if required.
Recording Injuries and Illnesses
Choose the Type of Injury or Illness from the provided options.
Select the affected body part using the diagram.
The front diagram appears reversed, like a mirror image.
After selecting a body part, additional checkboxes may allow for more detailed specifications (e.g., thumb, palm).
Selecting both the front and rear of a body part results in two injuries being recorded in the system.
Large-scale injuries, such as burns or long lacerations, may be considered multiple injuries by the system if marked on multiple body parts.
Treatment Details
If known, enter treatment details on the following screen. Administrators can later update this information for OSHA first report forms and record-keeping.
Contributing Conditions and Behaviors
Select applicable Contributing Conditions and Behaviors, which may be customized for your organization or chosen from our templates.
Attaching Photos
Launch the camera to take photos.
Use the Gallery function to upload previously taken images.
Adding Witnesses
Tap the green “Add Witness” button.
Enter witness details.
Capture an audio statement if necessary.
Obtain the witness’s signature within the app.
These steps ensure that all necessary information is accurately recorded for reporting and compliance purposes.
