These instructions are intended for those using the mobile version of our Safety Incident app. If you haven't already done so, please download the app (Android | Apple) to get started. The Incident app is compatible with both smartphones and tablets. Our app is also accessible from your computer via our website.
Signing In
Use your credentials to sign into the app from the Settings tab. After signing in, past incidents will be shown on the incident home screen. Color-coding allows you to see which have been submitted and which are in progress.
Creating a New Incident
Tap the green “Start New Incident” button.
Add an Incident Name and choose details about the incident from several drop-down menus.
Incident Name and Incident Type are required fields.
Incident Type options include:
Injury
Illness
Near Miss
Vehicle
General Liability
Property Damage
Incidents can be recorded for Employees or 3rd Parties.
Choose from among locations:
If locations were prefilled by your administrator, they may match ‘Companies’ if you also use Safety Inspection.
Otherwise, manually enter the location details.
If recording employee-related incidents, the Activity drop-down helps classify the nature of work (e.g., normal activity, overtime).
Some incident types allow you to Geo-Tag the location.
Entering Incident Details
Use the provided fields to describe:
What led up to the incident.
A narrative about the incident itself.
The Incident Description field becomes part of the official report.
Some accounts may have custom fields below the description. If required, these fields will be marked with an asterisk (*).
Selecting Contributing Conditions and Behaviors
Choose relevant Contributing Conditions and Behaviors from the list.
These may be custom to your organization or selected from standard templates.
This function is the same for Injury, Illness, and Near Miss incident types.
Adding Photos
Use the camera to take photos within the app.
Alternatively, retrieve photos from your device’s Gallery.
Adding Witness Information
Tap the green ‘Add Witness’ button.
Enter witness details in the fields provided.
Capture an audio recording of their statement (optional).
Collect their signature within the app.
By following these steps, you can efficiently document and report Near Miss incidents, ensuring accurate record-keeping and safety improvements within your organization.
