Safety Incident User Guide

This guide is designed for users of the mobile version of our Safety Incident app. If you haven't already done so, please download the app for Android or Apple to get started. The Safety Incident app is compatible with both smartphones and tablets and can also be accessed from your computer via our website.

Getting Started

Use your credentials to sign in to the app from the Settings tab. Once logged in, you will see past incidents on the Incident Home Screen. Incidents are color-coded to indicate whether they have been submitted or are still in progress.

To create a new incident, tap the green “Start New Incident” button. Enter an incident name and select details from various dropdown menus. The Incident Name and Incident Type fields are required, while others may provide additional useful details.

Incident Types

Incidents can be categorized under:

  • Injury

  • Illness

  • Near Miss

  • Vehicle

  • General Liability

  • Property Damage

Incidents may be recorded for employees or third parties. If your administrator has prefilled locations, they will be available for selection; otherwise, you can manually enter the location.

For employee-related incidents, the Activity dropdown helps classify the event (e.g., normal activity vs. overtime). Certain incident types may allow Geo-Tagging for location tracking.

Navigating the App

Each incident is structured into sections, with black arrow keys at the bottom to move forward or back through screens. Fields are available to document:

  • The events leading up to the incident

  • The incident description (which becomes the official report narrative)

  • Additional required fields (marked with an asterisk *)

Different incident types may have unique screens, diagrams, and data fields.


Incident Type Details

Injuries and Illnesses

  • Choose the injury or illness type.

  • Select the affected body part using interactive diagrams. (Note: The front diagram is mirrored.)

  • Use checkboxes for specific details (e.g., thumb, palm, etc.).

  • Enter treatment details, if known.

  • Administrators can update treatment details later for OSHA or recordkeeping purposes.

Additional Details:

  • Select Contributing Conditions and Behaviors (customizable by your organization).

  • Include incident photos (capture directly or upload from the gallery).

  • Add witness information via the “Add Witness” button. Witnesses can provide recorded statements and digital signatures.

General Liability and Property Damage

  • Specify whether the incident is for reporting only or linked to a claim.

  • Enter the incident description and claimant details, if applicable.

  • Claimants can also be designated as witnesses, pre-filling their information.

Near Miss Incidents

  • Describe the events leading to the near miss.

  • Select Contributing Conditions and Behaviors (same as in Injury/Illness incidents).

  • Attach photos and supporting documentation.

Vehicle Incidents

  • Enter details about:

    • Your vehicle (type, actions at the time of the accident, etc.)

    • Damages (photos and descriptions)

    • Other property damage (buildings, other vehicles, etc.)

  • Record conditions at the accident site using dropdowns and narratives.

  • Utilize the accident diagram tool to illustrate the incident, including objects like construction equipment and vehicles.

  • Use checkboxes and text fields to document unsafe driving behaviors.

  • Attach supporting photos.

Additional Vehicle Incident Features:

  • Add witness details using the “Add Witness” button.

  • Witnesses can provide statements via audio recordings and digital signatures.

The Safety Reports Incident System is a powerful tool for managing workplace safety and compliance. Additional features include:

  • Incident Investigation tools

  • NAICS Data Reporting

  • OSHA Reporting (including Forms 300 and 300A)
