Forming ‘Yes’ and ‘No’ Responses

‘Yes’ and ‘No’ responses should:

  • Restate the question while clearly addressing the inspector’s observation in plain text.

  • Ideally be typed as full sentences (no need for a period at the end).

A ‘Yes’ response indicates a positive observation (e.g., the absence of a hazard or issue).

  • Bad ‘Yes’ response: "There are obstructions in aisles."

    • This identifies a hazard, but the system counts it as a positive observation.

  • Good ‘Yes’ response: "The aisle is free of obstructions."

A ‘No’ response should correctly reflect a negative observation.

  • Bad ‘No’ response: "There are no obstructions in aisles."

    • While this notes a hazard-free aisle, the system incorrectly counts it as a negative observation.

  • Good ‘No’ response: "The aisle is not free of hazards."
