This guide is designed to help administrators build and conduct training exams using the Safety Reports Safety Training System. Whether you're using templated exams from the Safety Reports library or creating custom toolbox talk exams, this process allows for online creation, delivery via email or text message, and response tracking for review by trainers and administrators.
Building Exams
To begin, log into your account's admin page and follow these steps:
Navigate to Training Exams
On the left side of your admin page, select Training Exam.Create a New Exam
Click the green Add New Training Exam button on the right-hand side of the screen. You'll then be prompted to select the following:Training Source
Category
Individual Toolbox Talk (e.g., Aerial Lift training under the CPWR training source)
Customize Exam Name
The exam will automatically populate with the name of the toolbox talk, but you can modify it to your preferred title.Add Questions
Click on the Add Question link to open the Training Exam Question Dialogue box. Questions can be True/False or Multiple Choice with 2, 3, or 4 options.Type in the question text.
Choose the correct answer from a dropdown menu.
Click Add Question when done.
Save Your Exam
After adding all questions, click Update at the bottom-left of the Exam details section to save your exam.
Editing Exams
You can edit existing exams at any time by following these steps:
Find the Exam
Locate the exam you wish to edit and click the Edit icon on the far-right side of the row.Modify Title and Content
Edit the exam title in the Exam Details section.
Modify the exam questions under Exam Questions. You can also change the order of questions by dragging them up or down.
Save Changes
After making changes, click the Update button to save your edits.
Important Warning: If you change the answer key (e.g., switching which answer is correct), it will affect previously submitted exam results. This may cause previously correct answers to be marked as incorrect.
Using Templated Exams
Safety Reports provides templated exams for various topics. These can be used as-is or modified for your needs. To use a template:
Choose a Template
Select the training source, category, and title of the training in the dropdown menu.Save and Modify
You can modify the content, but ensure to click Update to save your changes.
Note: Only the most recent version of an exam will be sent if multiple exams are assigned to the same toolbox talk. To use multiple versions, duplicate the toolbox talk, modify the title, and assign the new exam.
Assigning Exams
To assign an exam:
Navigate to Training Events
Click Training Events on the left, then select Add New Training Event.Set Event Type
Choose either Future Event or Completed Event. Typically, exams are given after training, but they can be linked to a future event.Choose Course and Exam
Search for the toolbox talk by title, or manually select the source, category, and material. Select Exam Available if the exam is ready.Set Exam Date and Reminder
Set the exam's delivery date and the reminder date for completion. The default reminder is 72 hours, but this can be adjusted.Assign Recipients
Choose recipients by selecting their employee group or individual names. Ensure their email or phone number is entered to deliver the exam.Send Exam
Once all details are complete, click Send Training Test.
Delivering Exams
Exams can be delivered via email or text message. To manage preferences:
Edit Employee Profile
Navigate to Manage Employees and select the employee you want to update. Enter the email and/or phone number for the exam delivery.Choose Delivery Method
For email exams, select the email option.
For text message exams, enter the phone number and check the box next to Use for Exam Delivery.
Time Zone Settings
Make sure the employee’s time zone is set correctly, especially for text message exams, which are delivered between 06:00 AM and 08:00 PM in the recipient's time zone.
Taking a Training Exam
Once the exam is delivered, recipients will receive either an email or text message containing a link to the exam.
Email Recipients
The recipient will receive an email from reports@safety-reports.com. The subject line will feature the exam title, and the recipient can click on the Toolbox Talk Exam Link to access the exam.
Text Message Recipients
The recipient will receive a text from (402) 242-3220 with a link to the exam. Clicking the link will open the browser to the exam.
Note: Recipients may need to disable pop-up blockers or grant permission for mobile browsers.
Reviewing Results and Re-Sending Exams
Review Results in the Event
In the Completed Training Event, click the Edit icon to see results. The number of attempts, whether the trainee passed (100% is required), and options to resend the exam are available.Training Exam Report
Navigate to Training Reports and choose Training Exam Report to generate a report. Filter by employee, date range, or exam type as needed, then click Run Report.Download Report
Download the report in a spreadsheet format to view detailed results.
Troubleshooting Exam Errors
If a recipient encounters an error stating that the exam has expired or was deleted, contact your administrator. In case of any issues accessing the exam, make sure pop-up blockers are disabled or that mobile browsers have the necessary permissions.
