How to add a Forms User

Add users to the Forms module manually or by importing a CSV file from the admin side.

Please see the steps below to add a user to the Forms module. Users can be manually added or imported

Reminder: This can only be done from the admin side.

Add Training users as Attendees or Trainers from the admin side, or import users using a CSV file.

To add a user to the Forms module:

  1. Log into your admin account

  2. Click on the Forms module

  3. Select User Maintenance

  4. Click Create New User

  5. Add Required individual user details:

    • All required information:

      • First Name

      • Last Name

      • Email Address

      • Username

    • All optional information:

      • Title

      • Division

      • Cell Phone Number

      • Status

      • Notes

  6. If they are a new user, before clicking add/update check the Password Reset box

  7. Click Add/Update


Once you have added a user to any of the modules you will have the ability to import them from that module to any of the other modules.

To do that you will:

  1. Select the module you want to add them to

  2. Click User profiles, User maintenance, or Manage employees

  3. Click import users

  4. Select the circle next to Import from Safety Reports

  5. Check the box next to their name

  6. Click the Import selected users button


Additionally you may import users from a CSV file.

To do that you will:

  1. Click the Import Users Tab

  2. Check the import from CSV option

  3. You have the option to add them to a group by selecting Observation User Group

  4. Click the choose file button to upload a CSV

    • Please note: The users can be imported from a comma separated list (CSV).

      • All CSV files must contain a first, last name, and email. You can optionally include address, city, state, title, company, name, work phone and cell phone.

  5. Click upload once you have the file selected.

If a user already exists in the system and they are imported from another module, their password will carry over to any module they are added to.

Note: You must have at least one user entered into the system before importing others.


Good to know:

  • If this user is new to the system please make sure to go back to the User profiles and send them a password reset for any module that they are added to.

  • If a user already exists in the system, their password will carry over to any module they are added to.


⚠️ Important: This guide applies only to the Forms App in Safety Reports. The steps and features described do not apply to other apps, including:

Inspections App, JSA App, Observations App, Incident App, Incidents App, Safety Scan App, Training App.
